Requirements for Enrolment

Proof of Birth (either birth certificate or passport)
If child is born overseas, we must have date that the child arrived in Australia together with a valid visa/passport.
Imunisation certificate. This can be obtained from Immuniation Registry or from Medicare Office.
Medicare Number, Health insurance and Ambulance member number.
Family Doctor's details.
At least 2 emergency contacts.
Parent's work details.
